Procedures for disclosure of personal information
- SOFTBANK BB sends its ‘Application for disclosing personal information’ and the Guidebook with the address for sending back the application form to, fee amount and method of payment to an applicant who has requested the disclosure of information at the relevant contact.
The applicant who has received such application form shall pay the fee in the method of payment specified in the Guidebook and return the completely filled application form to the address together with ID verification document designated by SOFTBANK BB. If the application is made by proxy, such proxy is required to send also his ID verification document.
SOFTBANK BB starts procedures upon receiving the completed application form with ID verification document and confirming payment of the fee at the contact designated by SOFTBANK BB.
Procedures for correction of personal information and suspension of use of personal information
- In response to a request for correction to the relevant contact, if an applicant presents matters sufficient to identify the target contract and SOFTBANK BB deems that such correction is necessary to achieve the purpose of use, SOFTBANK BB shall make correction based on its judgment.
- In response to a request for suspension of use to the relevant contact, if an applicant presents matters sufficient to identify the target contract and SOFTBANK BB deems that such request is reasonable, SOFTBANK BB suspends such use.
Fee for disclosure of personal information and method of payment
The fee for disclosing personal information is 1,600 yen for each request.
The fee shall be paid as specified by SOFTBANK BB.
